HealthJay

HealthJay is a care engagement app that uses a Building-Block like interface offering person-centered access to healthcare recipients. Clinical care, chronic care, and social determinants of health are all delivered including walkie-talkie video chat with care providers. Users can manage care events in this ultra simple-to-use app using a whole-health approach. This gateway app takes care of recipients and care providers through care journeys with several transformative digital breakthroughs: • All communications are automatically translated into the users selected language with both text and high quality speech synthesis • The user chooses their preferred language from a selection of 25 languages • An optional “auto-join” feature allows for scheduled video wellness checks and appointments to be automatically joined, without requiring even a single button press • Tap on avatars to place audio and video calls, including group calls • Automated reminder and messaging systems for care tasks • Calendar for viewing daily events • Tap button to request help • Communities may create their own video libraries and class activity schedules, all browsable by language, category, and channel • Electronic visit and service validation
