Description
The myCareTC application assists patients in engaging with their healthcare providers. With this application, you’ll be able to:
· Submit daily check-in updates
· Exchange chat messages
· Monitor your vitals,
· Complete health assessments
· View health-related educational content
· Review your care plans and other summarized health information
The myCareTC app allows patients to take greater accountability for their health and wellness while increasing both communication and information exchange with their doctors and clinical care team members.
NOTE: this application is available to US-based patients only, as subscribed by their participating clinicians. Please contact your healthcare provider for more information.
